IMPORTANT: The advising questionnaire is one of the rare elements of your file that requires coordination with your chair/coordinator in advance. In the semester immediately preceding the semester in which you submit your evaluation materials), all advisees will fill out a questionnaire administered by the chair/coordinator concerning their advising experiences. It's recommended that you forward a complete list of your advisees and their emails to your chair shortly before Advising Week (spring semester for pre-tenure review, fall semester for tenure file and full professor file). Many departments already have templates for advisee surveys; if not, the VP for Academic Affairs' office can provide one, as well as offer advice on how to summarize the results.
For best results, chairs/coordinators are encouraged to send out the questionnaire the week after Advising, and to follow up twice (compliance is notoriously low, so don't panic). Note also that the candidate relies on the chair/coordinator to compile the data and anonymous comments into a summary, add his/her signature, and forward the document to you as a PDF for inclusion on this page. For this reason, it's smart to stay in contact with your chair as this progresses, and to coordinate a clear completion date well in advance of your file submission.

OS3 is acknowledging faculty who make active use of the Beacon Alert system with certificates. As the College recognizes advising work as part of teaching, this section would be appropriate to include such recognition from OS3 as an indicator of your collaboration with the larger advising network on campus.
