The completed application and a $35 background check fee (money order or cashier check) should be dropped off at the SLVHC office, located at 513 La Due Ave in Alamosa or at the property you are applying for.
Copy of picture ID, driver's license, or residency identification (we can make copies when you bring in your $35 background check fee).
You have the right to provide an official Portable Tenant Screening Report if issued within the past 30 days of this application submittal at no charge
Proof of income: most recent 3 months of paystubs or sign the attached EMPLOYMENT VERIFICATION REQUEST in the bottom box so we can obtain information from your employer. If not employed we need proof of income from other sources like TANIF, SSI, SSDI, Disability, or retirement. ALL INCOME FOR ALL HOUSEHOLD MEMBERS MUST BE PROVIDED.
Bank Statements: 6 months of bank statements for ALL accounts for ALL adults with your name on them. This includes accounts for children, parents, or others that have your name for various reasons and all online apps like Venmo, CashApp, and PayPal where you transfer money back and forth. These must be from the bank not screenshots.
