• Volunteer Character Reference Form (to be completed by someone over 18 who is not related to you) 

If you have volunteered with Saint Louis Public Schools in the past, please complete:

• Missouri Family Care Safety Registry Form

No matter which application you are using, please submit ALL forms together for the fastest processing!
