Volunteer Form Fillup
Initially, you will be registered as a volunteer. After some duration, depending on your activities, you will registered as a member of our family.
Rules for Registration (Must Read Before Form Fillup):
1. Only Volunteer can be enrolled using the form given in the below link. Being a member of this organisation is strictly subject to the discretion of the governing body members of this organisation.
2. Once a willing person fills out the form, he can further edit the form from the link given in the e-mail sent to him after submitting the form.
3. Only MALE candidate individuals can fill out the form as acceptance.
4. After filling out the form, acceptance of the individual as a volunteer of this organisation is subject to the decision of the General Secretary and if need then consulting with Governing Body Members.
5. A selection e-mail will be sent to the person who fills out the form once his volunteer-ship is confirmed by General Secretary.
After getting selection mail, the individual must contact to the General Secretary Swarup Dey (Mobile: 91266 10296) or Assistant General Secretary Abhishek Lahiri (Mobile: 8902382518 ) or In-Charge of Web & Communication Dept. Debmalya Santra (Mobile: 8348124947/ 9563625767).
Communication with this above mentioned person after getting e-mail is mandatory for all, who fills out the form.
No action will be taken regarding confirmation until the communication is made.
7. After the previous step, a hard copy of selection containing a ACCEPTED SEAL on behalf of this organisation will be sent by POST or HANDED over physically indicating a successful selection of the individual as a volunteer of this organisation.
6. Once a volunteership is confirmed, a volunteer can apply for a social service certificate from our organisation after citing proper reason of requirement(e.g. jobs, internships,etc.)
Requirements(for filling Volunteer Form):
1. AADHAR Card.
2. Photo (not bigger than 200 x 200 pixels). The photo can be resized using paint or online picture resizing tool available here
3. Signature (50 x 100 pixels),
4. E-mail ID (Mantadory),
5. Mobile/Telephone number.
6. Blood Group (Mandadory)
