ROJoson Outpatient Medical Records and Advices on Medical Record Keeping

It will be the policy of the ROJoson Medical Clinic to make an outpatient medical record for all its patients and a duplicate copy will be given to all patients seen. A fee will be charged for the duplicate outpatient medical records.

The patients shall bring the duplicate outpatient medical records issued to them whenever they consult again at the ROJoson Medical Clinic. These may be referred to during the current consult.

All patients should bring old medical records derived from consults with other physicians and other hospitals if they have done these prior to consulting at ROJoson Medical Clinic. Medical records include paper documents containing medical reports; x-ray plates; imaging pictures; medical tracings; photos; video recordings; etc.

All patients should develop the habit of keeping a complete file of their medical records. The file should be easily retrievable. As much as possible, they should have a back-up copy of their medical records kept in another place. As much as possible also, they should have digital copies aside from the hard copies. The digital copies can be scanned copies of their medical reports kept in USBs (preferably with a back-up) for easy retrievable and access by their current physicians.

The medical records should be made readily available to whoever physicians they are currently consulting.

These are important in helping the current physicians gather data, make an evaluation and decide on the treatment.
