Application and Evaluation Form Instructions
The PDK Student Teacher of the Year Award will be granted to the student teacher in each semester who best exemplifies the PDK vision of Service, Leadership, and Research. To participate, you must be a LIUPost School of Education undergraduate senior or graduate student in your student teaching year. Board members of PDK are not eligible.

Additionally, your Supervising Professor and each of your Cooperating Teachers must fill out and return separately the attached PDK Evaluation Form by the due date indicated below. A letter of support from your faculty supervisor is strongly recommended.
Award recipients will be notified by the end of Spring(Date: TBA) 2015, and honored at the annual Phi Delta Kappa Induction Dinner (Date: TBA).
Phi Delta Kappa is the premier graduate honorary society in professional education. Founded in 1906, it is international in
scope and membership. The LIU Post Chapter #1524 was founded in 1986.
