Application and Evaluation Form Instructions
Phi Delta Kappa Chapter 1524 is pleased to announce its first-ever student teacher of the year award. This award will be granted to the student teacher in each semester that best exemplifies the PDK vision of Service, Leadership, and Research. Participants must be a LIU Post School of Education graduate student in their student teaching year. Board members of PDK are not eligible.
Interested applicants should register with Professor Maria Zarycky, PDK Awards Director, via mail, in person, phone (516) 299-2895, or e-mail at email@example.com, and submit a Student Teacher of the Year Award application form.
Additionally, applicants need to provide the name and contact information for both their Supervising Professor at LIU Post and their Cooperating Teacher. An evaluation form will be forwarded to them. The Supervising Professor and the Cooperating Teacher need to return the completed evaluation on behalf of the student teacher directly to Maria Zarycky’s attention no later than the due date below.

Award recipients will be notified by the end of Spring(Date: TBA) 2015, and honored at the annual Phi Delta Kappa Induction Dinner (Date: TBA). They will receive a complimentary meal at this event, and a one-year paid membership to Phi Delta Kappa.
Phi Delta Kappa is the premier graduate honorary society in professional education. Founded in 1906, it is international in
scope and membership. The LIU Post Chapter #1524 was founded in 1986.
