Why This Form Matters
Helps keep Scouts and adult participants safe: Provides medical professionals with current health history, emergency contacts, and consent.
Required annually for all participants: Parts A and B must be completed and turned in by everyone (youth, parents, guardians, leaders) for any Scouting activity, even local or single-meeting events.
Strict confidentiality: forms are paper-only, kept secure, and shared only on a need-to-know basis.
Speedy & Stress-Free Tips
1. Download and complete the PDF form electronically (Parts A and B) for your Scout(s) + any parents/guardians .
2. Save a digital copy of each completed form to your files (e.g., “2025 Child/Parent’s Name AHMR Scouts Form”). In subsequent years, simply update any info that has changed!
3. Please be sure to complete all information fully and (except “High adventure base participants” section in upper right corner).
4. Print Scout(s) + parent/guardians forms and bring to next Pack meeting.
Protecting Privacy & Handling Forms
Forms are stored securely by designated leaders.
Forms will be shredded or returned at your request at the end of each academic year.
Medical details will only be shared with leaders who need the information for safety.
