Pre-employment or pre-placement health assessments can ensure fitness to carry out required tasks as well as safeguarding colleagues and clients.
This form is designed to assess if a candidate is fit to carry out the duties required within the new role.
In line with the Equality Act 2010 the medical questionnaire should be used after having provided a job offer to assess if the employee may have any specific needs and to comply with Health & Safety legislation to ensure safety for the new employee, colleagues and other people who may be affected.
The form can be handed to candidates for completion.
Should problems be highlighted the form can be forwarded to the address provided on the form for assessment by an experienced health professional. A fitness certificate will be provided after the assessment.
The form can be used more than once within your company and does not include costs for the medical assessment and certificate.
This form comes with £10 of vouchers. 5 vouchers of £2 each which can be redeemed against professional assessment of the completed form.
The form is provided as Word document permitting to be completed on the computer.
