Regular medical assessments can ensure fitness to drive and minimize danger.
This form is comprised of 2 parts. Part 1 is to be completed by the employee and part 2 can be completed by the employee's GP or practice nurse at their local surgery. The GP may charge you a fee for the medical. The form does focus on the relevant examinations and therefore prevents gathering of unnecessary information.
Should problems be highlighted the form can be forwarded to the address provided on the form for assessment by an experienced health professional. A fitness certificate will be provided after the assessment.
The form can be used more than once within your company and does not include costs for the medical assessment and certificate.
This form comes with £10 of vouchers. 5 vouchers of £2 each which can be redeemed against professional assessment of the completed form.
Costs £20.00 + VAT for internal use within your business. Discount rates available if bought in conjunction with other occupational health services.
