In accordance with the Working Time Regulations employees who work regularly nights require a health assessment at regular intervals. Night work means working for at least 7 hours a night with and minimum of 3 hours between 23.00 and 06.00 on a regular basis.
The night worker health questionnaire covers all the relevant health aspects regarding night work. By focusing on questions related to night work, the form respects employees' right of privacy and compliance with Human Rights is ensured.
The form can be handed to employees working nights for completion.
Should problems be highlighted the form can be forwarded to the address provided on the form for assessment by an experienced health professional. A fitness certificate will be provided after the assessment.
The form can be used more than once within your company and does not include costs for the medical assessment and certificate.
This form comes with £10 of vouchers. 5 vouchers of £2 each which can be redeemed against professional assessment of the completed form.
The questionnaire is provided as word document and can be completed on the computer.
