Working in noise requires regular medicals due to legislation. The Health and Safety at Work Regulations require safeguarding employees from unnecessary and loud noise. The employer has to provide special noise medical before start and then at regular intervals (usually yearly).
This form is comprised of 2 parts. Part 1 is to be completed by the employee and part 2 can be completed by the employee's GP or practice nurse at their local surgery. The form does focus on the relevant examinations and therefore prevents gathering of unnecessary information.
Should problems be highlighted the form can be forwarded to the address provided on the form for assessment by an experienced health professional. A fitness certificate will be provided after the assessment.
The form can be used more than once within your company and does not include costs for the medical assessment and certificate.
This form comes with £10 of vouchers. 5 vouchers of £2 each which can be redeemed against professional assessment of the completed form.
