School Affidavit Application Steps and Forms Needed
The following required documents will need to be completed and turned in:
Affidavit for School License
The Affidavit for School License is available on the Iowa DOT website. The student will need to print out this form and complete it.
Driver’s Permit:
Bring the original or a copy to the District Administration Office; student must have held permit for at least 6 months. No exceptions.
Next steps:
The student must live at least a mile from the school - no exceptions to this rule. The district will do a Google Maps search to ensure that the address listed follows the guidelines.
Once the completed form and permit documentation is satisfied, Dr. Tate will sign the form. If he is not immediately available, building secretary keep the form and call the student when it’s approved.
Final step:
The student goes to the Driver’s License Station to be issued a new license. They will be expected to show all of the above completed documentation plus proof of completion of driver’s education. The AEA inputs all completion information to the DMV electronically following the completed class.
The cost is $8. The parent is required to complete page 2 of the Affidavit – if that is done, they don’t have to accompany the student to the Driver’s License Station.
