General:
1. Do I really have to click 'Start Exam' prior to seeing a patient?
Yes. This satisfies a CMS regulatory measure. Signing up for a patient is not sufficient.
2. Where is 'Reg Now'?
It's gone. Registration will be able to see who has been set for discharge by filtering their trackboard.

Form Fast will continue to exist. It will be melded with EPIC at some point.
7. What will happen to the Radiology discrepancy workflow?
Nothing changes except that the PA should enter an addendum to a patient's chart. There will be far fewer with CTs now being read as final.
8. Is the workflow for patients being referred in any different?
Yes. You will enter the preliminary information into a note that will then be merged with the patient chart by registration , including the note.
9. STEMI process. Open up the 'chest pain/ACS' order set, scroll down to the STEMI section, and click the orders you want.
10. Do I need to place a consult for hospitalist admissions? No.
Clinical Documentation and Medical Decision Making
1. I can't find a template for my chief complaint?
Use General Illness. There are many more templates that will be added in 2 months (altered mental status, etc.)
2. Where do I document my personal read of an imaging study?
Under the 'Results' report (that shows up when you click a patient in the tracker), next to the imaging there will be an option to 'Read'. This will enter the formal chart. For EKGS, please use a templated procedure note for interpretation.
3. There is no place to document who gave the history?
CMS now wants us to document that 'The wife states..'. It's not enough to say that the wife helped provide history. Be sure to include this in your documentation.
4. Be sure to click that you reviewed the labs or imaging tests in the MDM tab.
5. How do I place a critical care note?
It is now listed as a procedure note. Please do not use a smart text.
Orders
1. Do I need to place an order for lidocaine, gelfoam, etc. that a nurse gets from the pyxis?
Yes. This is a change from our current workflow.
2. When giving verbal orders, you will need to provide the nurse with your enterprise ID. It is five digit number that you will be receiving shortly.
Admission
1. How long before a patient's inpatient orders are released?
Immediately. As a physician, you must release them.
2. Is there a different place to enter the bridging orders?
Yes, this should be done within the Admission Navigator that you will be filling out. Be sure to 'Sign and Hold' not sign.
3. How do I release admission orders?
a. Go to orders
b. Click on the 'Signed and Held' tab
c. Click on 'Select all....'
d. Click on 'Release Orders'
4. How do I designate Call Made?
You must now order either 'Decision to Admit' or 'Decision to Place in Observation'. This will serve as a)indicator that the call to an admitting attending has been made 2)initiates a bed request within the hospital and c)satisfies the CMS measure
Discharge
1. How do I e-prescribe?
The software will default to printing prescription unless you change it. The local pharmacies are all loaded. We are working on which individual will be responsible for entering this information. If it isn't filled out, you may enter it yourself.
Credit Scenarios:
1. How do I get credit for signing a PA chart?
You must co-sign their chart. Add an attestation note, a major tab seen on the left sidebar if you see the patient.
2. How do I get credit for disposition?
Add a progress note. Use the SmartText .eddispo to populate the standard language necessary. You may add other comments to describe
3. Doctors, do not enter a midlevel's note and sign their chart. If you need to document, place an attestation note.
MyChart
1. What will a patient see in MyChart?
Only labs and test results, not your note. The results will autorelease in 3 days.
2. How do you release the results early?
Click on 'More Activities'. Then click 'MyChart Utilities'. Finally, click 'Rstls release'
Meditech
1. How long will Meditech exist?
The past information in PCI will be available to you indefinitely, as of now. You may enter progress notes, etc within Meditech up to 15 days go-live.
