Risk Assessment and Acknowledgement Form

The Fieldwork Risk Assessment is the main document you will need to read. To access it, go to the Health & Safety blackboard site and click on the 'Risk Assessment and Acknowledgment Form' folder (see image below).

Once you are in the right folder, you need to download the risk assessment and read it. To download, you need to click on the text inside the red box on the image below.

Once you have downloaded the Risk Assessment, you need to fully read it and then complete the acknowledgement form

To access the Acknowledgement form, you need to click on the acknowledgement form link below the Risk Assessment (see image below).

Once you click on this link, you should see the screen below. You should then click on Begin on the bottom right to begin the form. Please note that although this is listed as a test, you are not being marked or assessed on this. It will not effect your grade in any way.

Once you click begin, you will see the screen below. You must agree to all the terms in questions 1 -4. The answers underlined in red are what you must select. Once you have selected the answers for questions 1-4, you must click 'Save and Submit' in the bottom-right corner.

Once you have done this, you will be able to go out on Fieldwork. In order to access Media Store equipment, please go onto the next step, which is completing the Video Quiz.
