Search for a student that you are wanting to evaluate. Beside the Student’s Name, click the “Documents Icon” (looks like three lines)
Hover over, “Student Information” and from the drop-down menu, select “Referral Information.” Next, click “Add New Referral Information” and add the all information, then click “Accept”.
Once this is done, click “Documents” beside “Profile” to create the “Notice of Evaluation”.
Note: Do not put information into the Parent Information from Notice of Evalaution/Reevaluation Documents. If you put information here prior to creating the Notice of Evaluation/Reevaluation you will get an error message.
Keep the consent form in “Review” until you receive the parent’s response.
Parent response will automatically flow back to the “Referral Information” section under “Student Information”
Evaluations can be written and finalized directly in the system
Example: If evaluating for DD and Speech
Speech can write and finalize their report in the system
The DD Evaluation can be uploaded separately
All evaluation reports should be finalized and uploaded before the conference.
