A yearly physical examination is required for interscholastic participation. Students planning to participate in athletics are required to obtain their own physical examination prior to participation. To avoid problems with summer physical appointments, it is suggested that the student-athlete schedule his/her physical in the late spring and submit the results the school before the close of school in June.

The Seymour Physical Form must be completed by a physician and submitted to the school nurse prior to participation. The cost of the examination is the responsibility of the student/athlete.

The physical form and all other information must be uploaded and completed in Family ID before a student-athlete tries out for a sport.

It is important to note that NO athlete will be permitted to participate (try out or practice) without meeting these requirements.
