Behavior services can be provided as an intensive ABA program or a consult model to either ages 0-3 or ages 3+.
Age 0-3 intensive programs should address all developmental areas.
Age 3+ intensive programs are focused more on behavioral needs. The expectation is that generic resources such as school should be addressing cognitive and motor needs. Over age 3 intensive ABA also requires denial from insurance prior to funding.
All age consultation, or behavior modification is time limited and only addresses one or two concerns. It does not usually require an assessment, just a report upon completion.
You can apply for any or all of these models under the 612 or the 620 service code.
612 service code is the most common behavioral service code , and refers to having a BCBA as the supervisor
620 service code is a supervisor who is a licensed professional such as a psychologist, LCSW, LMFT, or other applicable license and also has appropriate applied behavior education units and experience
613, 615, and 616 are staffing level subcodes that can be part of a 612 or 620 program
620 and 612 are separate applications and program designs. If applying for both, you will need 2 separate applications.

In addition to the general provider requirements HERE, behavioral services must also meet the following requirements:
Providers must possess the license, certification, and/ or credential as required by the State of California to practice in the field being offered.
Behavior services are primarily in home, working directly 1:1 with the client, or consulting with the caregiver on behavioral strategies. Caregiver attendance and observation is required for ABA programs
Services should not be provided in a clinic or daycare setting unless approved and documented by the planning team

Please contact your resource coordinator if you have any questions about the service implementation or application documents prior to submission. When submitting, all documents should be fully filled out, include all requested attachments, signatures, and dates, and be in PDF format
After March 1, 2026 all applications for service code 612/ 620, and related staffing codes associated with the same practice will go through the standard vendorization process. The steps can be found HERE.

Please do not combine 612 and 620 in one app/ PD. The 613, 615, and 616 service codes do not need a separate application, they will be added under your primary vendorization per your staffing levels needed.
612 and 620 services require a program design for each service. The program design checklist can be found HERE.
As stated above, you can choose to provide intensive, or consult for ages 0-3 or ages 3+. When writing your program design, please specify which programs you are applying for on the title page, as well as clearly delineate any differences throughout the program design in curriculum, hours provided, outcomes, and entrance and exit criteria.
You will also need to add a staffing addendum to specify the programs and staffing levels you will be using. The addendum is HERE.
You may use this as your job descriptions within the program design, but if your job titles or descriptions vary from the state requirements, please expand upon this within the job description section. Your minimum requirements must meet the minimum requirements within this document
Please submit a sample report for each service you are applying for. Sample reports should be written as if it is a real person. Please make sure there is no identifying information such as name, birthdates, or parent information. You can create your own template or use one of our sample templates. Please make sure you change the wording to reflect how you would write a report for an actual individual.
