Rapides School-Based Health Center offers services to Rapides Parish students with no out-of-pocket cost to the student or Rapides Parish School Board. Rapides Community School-Based Clinic offers all students urgent care, primary care, and behavioral health. Our purpose is to keep students in school, and parents at work, build healthy relationships and know what students need.
Wellness checkups are essential for a child’s growth and health. The exam addresses a child’s physical health, development, behavior, and general well-being. A wellness visit includes physical measurements, patient history, hearing, vision, anemia check, physical exam, developmental assessment, and a risk assessment. You will receive a letter after the visit detailing our findings.

Immunizations Louisiana Revised Statute 17:170 and Louisiana Administrative Code, Title 51, Section I-701
In order for students to enroll and remain enrolled in elementary or secondary schools, Louisiana state law requires documentation of certain required immunizations or a written exemption.
We will send an additional consent form home if your child is due for any immunizations per the CDC and Pediatric Guidelines. This consent form will indicate which immunizations are expected and will ask your permission for us to administer the immunizations.
