Incident Reporting

Any unexpected or unplanned incident occurring during clinical training that results in patient injury or which may result in patient injury should be reported. Incidents occurring in conjunction with the delivery of any patient care, that result in real or potential injury to patients, MUST be reported to the program administrators within 24 hours of the incident or discovery of the incident. Regardless of whether the issue seems litigious or not, program administrators are to be informed of the situation and the details so they can assess whether or not risk management at RFUMS should be notified.

Examples include, but are not limited to: traumatic intubations, dental injury or damage to bridges, crowns or caps, corneal abrasions, nerve or positioning injury, wrong blood type administered, patient fall, medication or antibiotic administration error, allergic/anaphylactic reaction, wet taps, aspiration, intraoperative myocardial infarction or patient death. If there is any question if an incident should be reported, it should be reported. The necessary forms are located online for the SRNA to download if needed.
