This form is to be completed by the driver and/or aide who witnessed the behavior.  It should not be completed by a third party.

This form should be completed for any of the following or any other serious incident:

 Assault 

Rude/discourteous/annoying behavior 

Bullying 

Smoking 

Destruction of property 

Yelling/inappropriate gestures out bus window 

Disobedient behavior

Spitting 

Disturbing others 

Sticking head/hands out of window 

Eating, drinking, littering 

Tampering with bus equipment 

 Fighting, Pushing, Tripping 

 Throwing objects out of window 

Failure to remain seated 

Unacceptable language or gestures 

Refuses to wear seat belt 

 Unusual loud talking or undue noise 

 Possession of a weapon 

Violation of safety procedures 

Possession of alcohol or drugs 

Other
