Nurse Information

NURSE

For your benefit we do have a full time nurse on staff.  The nurse is available to tend to those in need of medical attention and to counsel students on health problems.  A student may request a pass from his/her teacher to visit the nurse. If a student is too ill to remain in school, the nurse will make arrangements for an early dismissal.  At no time may a student be dismissed without prior parent authorization. Students who take it upon themselves to leave or have parents call after the fact, will be considered cutting.  Remember, the nurse is the only person who may excuse you from school for illness.

GYM EXCUSES:

A student with a medical excuse from a private physician must bring the note into the school nurse following the visit. An extended gym pass will then be written and the student will attend study hall. **Follow-up from the physician is required at the request of the nurse. If follow up is not obtained, points will be deducted from the student's gym grade each week until the note is obtained. *** A student that is medically excused from PE shall be excluded from extracurricular activities and may not rejoin that activity until medically cleared by the physician.

 Please observe and follow the following symptoms that indicate a student is too sick to attend school:

• If your child has any of the symptoms below, please keep him/her home. This is to help protect students from spreading communicable diseases. In order to protect other students and employees, it is important that students are kept home when they are too sick for school. 

• It will become necessary to pick your child up from school as soon as possible if he/she demonstrates any of the following symptoms while attending school:

• Fever – Temperature of 100.5 degrees F; or higher. Student may return when fever-free for 24 hours (without the use of fever-reducing medicine)

• Vomiting – A student should not return to school for 24 hours following the last episode.

• Severe Diarrhea

• Persistent cough that is disruptive to class.

Please note****Minimal pick-ups from the nurse will occur during 1st block. If the student comes to school not feeling well, they will be encouraged to return to class and then will be sent home after 1st block, at the Nurse's discretion. 

No student may sign him/herself out due to illness without a parent note. If your child is a driver and you permit them ​to leave school, then an email MUST be written to permit them to leave.  Please send to this address: pmhsattendance@pv-eagles.org  

MEDICATIONS: 

Medications can only be administered during the school day with an order from your physician and a parent/guardian co-signature. This policy applies to both prescription and over-the-counter medications. The student may carry only medications ordered by your doctor that can be self-administered by the student, such as asthma inhalers or Epipens. All other medications are to be locked in the Health Office. The doctor’s order must be renewed each school year. The medication forms are located on the PV website.
