Please upload your athlete's physical directly to your registration. We will no longer be accepting hard copies or emailed physicals.
FAQs:
*We are required by law to use the date of the exam on the physical, NOT the date of doctor's signature. Your physical is required to be on the specific, linked form above to be in compliance with OSAA policy.
*Fee payment is required BEFORE your athlete participates in any PIL sport - however, no sport at Lincoln will allow cost to be a barrier. If you are on free and reduced lunch or need a scholarship for participation, please reach out to our Bookkeeper, Anita Agudelo at Aagudelo@pps.net.
Need a summer physical? Some of the Multnomah County Student Health Centers are open and ready to help - see the flyers to the right. Make sure to call and make an appointment.
Don't wait until August to do this as health centers and doctor's offices get booked up!
