HEALTH & WELL-BEING PROGRAM

Actions Required to be Eligible for the 2024 Health Insurance Premium Discount / Wellness Rate

Pitkin County provides an opportunity for employees to save on health insurance premiums by completing the Employee Health and Well-Being Program. This program is voluntary. However, to earn the 2024 Health Insurance Premium Discount / Wellness Rate, employees and their covered legal spouse/partners must both complete each of the below outlined actions between November 1, 2022 - October 13, 2023.

1)  Complete a Primary Care Preventive Wellness Visit.

Report your visit here: PCP Visit Form

This includes medical and well-women exams at your primary care provider (PCP) or women’s health provider, not a specialist (eg. Cardiologist).

Preventive care is paid 100% by the health plan when following plan guidelines. Pitkin County’s CDHP medical plan covers Tier 1 (VHA Primary Care Network) and Tier 2 (Cigna OAP In-Network) preventive care at 100%.

Allegiance, Pitkin's medical claims processor will report quarterly on completion of exams. We are asking that you also self report via the link above.

Pitkin County does not receive any personal identifiable health data from the reporting. Pitkin will only receive a list of names of those that completed their visit for credit for the health insurance premium discount.

Pitkin County will continue to get discounted labs with Aspen Valley Hospital and MidValley Family Practice as part of the preventive wellness visit with Valley Health Alliance (VHA) providers. Certain labs are free to employees and their spouses that are on the health insurance plan.  Your PCP will review and recommend what labs are ordered. If certain labs are needed (CBC w auto, A1c, TSH, Chem13, PSA, Cholesterol, HDL, LDL, Triglycerides, and Glucose) then these labs will be paid for by Pitkin County, and not processed via your health insurance plan when drawn at an Aspen Valley Hospital (AVH) or MidValley Family Practice lab.  Your VHA PCP must use the VHA Client Form to order these free labs. If these labs are drawn at a lab other than an AVH or MidValley Family Practice lab, you may be exposed to additional out-of-pocket costs.  If you need additional labs, your PCP will schedule those using another lab order form, and those labs will be billed via your health insurance.  If you don't go to AVH or  MidValley Family Practice  for the free labs, then all labs will be billed via your health insurance.

2)  Complete a Preventive Dental Care Visit and Exam.

Report your visit here: Dental Visit Form

This includes an oral exam and cleaning with your dentist.

Our medical claims processor Allegiance will report quarterly on completion of preventive dental exams.  We are asking that you also self report via the link above.

Pitkin County does not receive any personal identifiable health data from the reporting. Pitkin will only receive a list of names of those that completed their visit for credit for the health insurance premium discount.

3)  Attest to being tobacco-free or complete a Tobacco-Free Success Plan.

Complete your attestation form here.

Attest to whether you are a tobacco user; indicate yes/no.

We want to support you in your journey to quit tobacco use. If you are a tobacco user, you can earn the same incentive by completing a Tobacco-Free Success Plan. There are several options available on the Tobacco Free Workplace page.

4)  Complete the VHA PCP Assignment Form

Important questions you may have...

The PCP Preventive Care Wellness Visit, and review of health screening results can be done via telemedicine depending on your provider.  Ask your PCP office if that's an option for you when making your appointment if you prefer a virtual visit.

If you and your PCP decide that you don't need labs this year, then you do not need to get labs for the Employee Health & Well-Being Program. You do need to complete the Preventive Care Wellness Visit.
