Physical Education Waiver
Students who participate in 2 seasons full seasons of a sport between 9-12th grade are eligible to waive the PE (.5 credit) requirements for graduation (pending verification of attendance to games and practices).
(Student cannot combine 1 PE course with 1 PE waiver. Two waivers MUST be completed in order meet graduation requirements; 2 waivers cannot be from the same school year)
Student is responsible for completing the PE waiver form after a season is completed.
(Signatures needed; Coach or Band Director, Assistant Principal and Counselor)
PE waiver forms must be turned in prior to senior year or PE courses will be added to student schedule. Forms are available online under Athletics or the Department of Teaching and Learning or here. They are also available in the Student Services Office of each high school.
Student Work Permit
Students requireing a work permit due to age can download the above form, have their prospective place of employment complete it, and their physician. Turn in the completed form to the Records Clerk in the Main Office.
Transcript Release Form
Students must complete a Transcript/Consent for Record Release form in order to have an official transcript sent to any college, company, agency or individual.
NOTE: All SAT and ACT scores are part of the high school transcript.
A parent's signature is required for individuals under the age of 18.
As of April 2020, this transcript request form can now be filled out digitally and can be emailed to each individual building's records room. Please fill out the form and email to one of the email addresses below:
Extended Absence Form
Students who plan to be absent from school for an extended amount of time must complete and turn in this form 5 days prior to departure. It is the student's responsibility to obtain signatures from teachers and missing assignments.
