Artifact 9: revised feeding therapy intake

At our practice, both occupational therapists and speech-language pathologists are involved in treating children with feeding, eating and swallowing deficits. Prior to attending the complex feeding course, my speech-language pathologist colleague and myself, recognized our current intake forms did not appear to be sufficient in gathering the information needed to make recommendations, particularly for clients seeking intensive feeding therapy. We often experienced children were not being referred to the most appropriate discipline and/or combination of modalities and approaches available. Following our completion of this course, we collaborated to develop a revised feeding therapy intake form that supports our ability to decipher role delineation when making recommendations for therapy.
