Check for the correct equipment, manuals, and forms as needed to ensure all items are present. Inform the Front Desk or Clinic Admin Team (nyuspeechclinic@nyu.edu) of any damaged or missing components.
Scan the QR code (on the label) using your phone or another device.
**Please make sure you are logged into your NYU account on your device. You will not be able to check out materials if you are not logged in.
Complete the "Clinic Material Check Out Form." The Equipment Name will be filled in automatically. Add the name of the student clinician who is checking out the equipment and select the reason for check out. Once all information has been added, submit your response.
You can double check the "Clinic Materials Inventory" sheet (linked above) to ensure you checked the equipment out correctly.

Scan the QR code (on the sticker) using your phone to open the Check Out form.
Once the form has loaded, go straight to the "Submit" button. Do NOT add any additional information to the form.
Double check that the equipment was checked in correctly using the "Clinic Materials Inventory" sheet (linked above). Find the equipment name and confirm that the Status reads "In."
The Department has a number of tests and materials available to students through the QGlobal site. Access is provided to students enrolled in Practicum I and II.
