Sylvan Lake and Red Deer
ICC is a rural application of the Longitudinal Integrated Clerkship model. I completed the equivalent core learning objectives for Family Medicine, General Internal Medicine, General Surgery, Obstetrics and Gynecology, and Psychiatry.
A colleague and I created a community initiative that proposed a relationship between a local community collaboration focused on social risk mitigation and the local family medicine clinic we were working with. The support agencies were focused on supporting vulnerable individuals/ families int he domains of food, housing, security, transportation, and youth/ families. We demonstrated the value in working with an interdisciplinary team of local social service support representatives to better address the social determinants of health by referring vulnerable families/ individuals within the community.
