Senior Green Folder: What's Inside?

At the beginning of the year, each of you should have received a Green Folder. In that Green Folder are forms made by your CCC Coordinators meant to make your post high school planning a bit easier!

Each of these forms exists here digitally! If you lose a form or simply need a fresh copy, look no further! Simply hover over the form you need and click the icon that appears in the corner to be brought to the page. If you need a new copy of a form, and have access to a printer, you're welcome to download the forms and print.

First Thing's First: FERPA

Before an RHS Counselor is able to release ANY of your information you--and your parent, if you're under the age of 18--MUST complete a FERPA Form. FERPA stands for the Family Education Rights and Privacy Act, a law that ensures none of your personal info is released from the school without your consent.

This year, you and your caregiver will complete the FERPA by reading and signing the "Consent to Release Educational, Free and Reduced Lunch Status and Financial Aid Information" form (this is the gold form in the front of your Senior Year Green Folder) and turning it into the Counseling Office.

Again, this form MUST be completed in order for RHS to release ANY of your information! You only need to complete the form once! You can complete it right here in the space below!

This year, transcript requests will be handled by filling out rows of the Transcript Request Form included above. Fill out the form and take it to your Counselor. The first 5 transcripts are free, and everyone after that is $2.

Remember, AN APPLICATION IS NOT FINISHED UNTIL A TRANSCRIPT IS SUBMITTED!

Letter of Recommendation Request

This is a handy form to complete when you're seeking a letter of recommendation. Fill the form out and give a copy of the completed form to the person you've asked to write your letter. They can use it as a "cheat sheet" when crafting your letter ;)
