Transition services refer to a coordinated set of activities that are focused on improving the academic and functional achievement of students to facilitate their movement from school to post school activities. Transition services are based on student plans for the future and include their Preferences, Interests, Needs, and Strengths (PINS).
Beginning not later than the first IEP to be in effect when the child turns 16, or younger, if determined appropriate by the IEP team:
Students should be invited to participate in their IEP meetings
Appropriate measurable postsecondary goals based upon age appropriate transition assessments related to training, education, employment, and where appropriate, independent living skills should be developed collaboratively with the student
The transition services (including courses of study) needed to assist the student in reaching those goals should be planned
Beginning at least one year before a student reaches age 18, or when the district obtains actual knowledge that within one year the student will marry or become emancipated before age 18:
The district informs students that procedural rights will transfer to them upon age 18, marriage or emancipation, whichever occurs first.
Our district's obligation to provide a Free and Appropriate Education only ends when a student has obtained a standard diploma or the end of the school year when the student turns 21 years old. Our Molalla Transition Program (MTP class website: here) offers a variety of services to youth who have obtained a modified diploma or whose IEP teams have decided transition program services are necessary in addressing short- and long-term post-school goals. The services provided by MTP include, but are not limited, to the following:

In the Spring of 2015, Jollene Hall, Michael Salitore, and Cheryl Renton applied for, and eventually obtained, a Youth Transition Program grant. Since that time, our district has been increasingly focused on enhancing our programs and transition focus to increase the number of students with disabilities who graduate from high school and leave our Molalla Transition Program with the knowledge and skills necessary for college and employment.
