Some MRSD staff have been trained in the Nonviolent Crisis Intervention (NCI) program, developed by the Crisis Prevention Institute. Any use of physical restraint or seclusion implemented by school personnel should follow NCI rules.Â
On the same day a restraint occurs, a Physical Restraint Incident Report should be completed. You can complete the form below or you can use the link: https://docs.google.com/forms/d/1e-6vCZx4aSUd3OQTRVMrVCMyCnwL8bEs8UBhBTXblMM/edit. *You must be logged in to Google to complete the form
Within two (2) school days of use of physical restraint, a documented debriefing by appropriate staff, including staff involved in the restraint must occur and Physical Restraint Incident Debriefing Notes should be completed. The purpose of the debriefing is to review the incident and take any necessary actions to reduce the chances that such an incident will reoccur. Those attending the debriefing meeting shall have the opportunity to review the Physical Restraint Report documenting the incident.
For both forms (Incident Report and Incident Debrief), three copies are delivered: (1) to the building administrator, (2) to the parent, and (3) to supported education office manager (for the file).
