Injury Reporting

All injuries—whether they occur on campus or at clinical sites—must be reported, including those involving faculty, staff, or students. 

Students are not permitted to self-report injuries. Instead, the supervising faculty or staff member, or the individual who witnessed the incident or received the report from the student, is responsible for submitting the injury report on the student’s behalf.
