MyOPAC文献取り寄せサービス利用申請

• 医学図書館では、以下の部局に所属する本学の学生・教職員からの申請を受け付けています。
  The Medical Library accepts applications from UTokyo students / faculty members who belong to the following departments: 

  • 医学系研究科・医学部
    Graduate School/Faculty of Medicine

  • 附属病院
    The University of Tokyo Hospital 

  • ニューロインテリジェンス国際研究機構
    IRCN: International Research Center for Neurointelligence

  • 保健センター
    UTokyo Health Service Center

  • その他、医学部・附属病院の建物内にある研究室等
    Laboratories etc. of the other faculty located in the Faculty of Medicine or the UTokyo Hospital

• 文献（複写物・図書現物）の取り寄せにかかる料金の支払いは、以下のいずれかで承ります。
  You can choose to pay the fee for ordering document deliveries at the following expense: 

  • 私費（銀行振込による前払い）
    Private expense (bank transfer, prepayment)

  • 公費（大学運営費等の四半期振替）
    Public expenses (e.g. University operation fee, quarterly transfer)

• 現在、カウンターでの現金でのお支払いはお受けしていません。私費の場合、お支払い方法は銀行振込による前払いとなります。ご了承ください。
  Currently, we do not accept cash payments at the counter. When you pay by private expense, the method will be advance payment by bank transfer.

  • 文献・図書を取り寄せる＞受け取り／支払い

• 利用料金を医学図書館が立て替え、四半期毎に経費の振替手続きを行います。
  The Medical Library will reimburse the fees and transfer the expenses quarterly. 

振替スケジュールの例：
Example of transfer schedule: 

• • 2020年度第１四半期（2020年4月～6月）利用分⇒2020年7月に振替手続き
    Fees during the first quarter of 2020 (April-June 2020) ⇒Transferred in July 2020. 

• 支払い予算に関する各項目が不明な場合には、ご所属の講座・教室の予算管理事務担当の方に確認してください。
  If you are unsure about each item about payment budget in the application form, please check with the person in charge of budget management of your course/laboratory. 

• ご記入の情報に基づいて振替請求を行いますので、間違いがないようご注意ください。
  Please be careful not to make any mistakes as we will make a transfer request based on the information you provided. 

• 申請時に記入された予算については、教室主任もしくは所属の長/予算管理者から事前に利用の了承を得ているものとみなします。
  The budget entered will be considered to have been approved to use in advance by the head of laboratory or the manager of the budget. 

• 大学運営費は、ご利用いただけます。
  You can use the university operation fee. 

• 大学運営費以外の予算（寄附金・共同研究費）を利用される場合には、医学図書館の大学運営費に「文献複写料金」として振替が可能かどうかを、ご所属の講座や教室事務担当の方にご確認ください。
  If you would like to pay with a budget other than the university operation fee, such as donations and joint research expenses, please check with your course/laboratory clerk if the budget can be transferred to the operation fee of the Medical Library as "document/photocopy fee". 

• 科研費は、ご利用いただけません。
  The budget of Grants-in-Aid for Scientific Research(KAKENHI) can not be used.

• 新規登録を希望する方、予算情報の変更など登録情報に変更が必要な方も、こちらのフォームからお申込みください。
  If you wish to newly register, or if you need to renew your registered information, please apply from this web form.

• このフォームは、図書館利用登録とMyOPAC登録の共通の申請フォームです。
  This web is a common application form for using libraries and MyOPAC delivery request service.
