図書館利用登録

• 医学図書館での登録は、以下の所属の学生・教職員（在籍者）が対象です：
  For UTokyo students/faculty members of the following ONLY: 

• • 医学系研究科・医学部
    Graduate School of Medicine/Faculty of Medicine

  • 附属病院
    The University of Tokyo Hospital

  • ニューロインテリジェンス国際研究機構
    IRCN: International Research Center for Neurointelligence

  • 保健センター
    UTokyo Health Service Center

  • その他、医学部・附属病院の建物内にある研究室等
    Laboratories etc. of the other faculty located in the Faculty of Medicine or the UTokyo Hospital

• 登録期限が切れた後に、引き続き本学に在籍される場合は、登録更新が可能です。
  Please apply for renewal of your registered information if you will continue to be enrolled in the university beyond the original expiration date. 

• 登録の際に設定される期限は、本学との雇用・在籍状況により各自で異なります。
  The expiration date is set according to your employment and enrollment status. 

• ご身分の変更、ICカード身分証の紛失・更新などの場合も、申し込みフォームから「登録更新」のお申込みをお願いします。
  Please apply to "Renew" from the web form also when your status was changed, or when you lost or renewed your IC card ID. 

• ICカード学生証を持っていて、かつ、東京大学学務システム(UTAS)にログインができる学部生・大学院生の方は、新規登録・更新ともに申請手続きの必要はありません。
  If you are a student and have your IC card student ID (undergraduates and graduates who can log into UTAS), you do not need to apply for new registration or renewing your registration information.  

• UTASに登録された情報が図書館業務システムにも反映され、お手持ちのICカード学生証で各図書館・室が利用できます。
  The information you registered in UTAS is reflected in the library system. You can enter any library with an IC card student ID that you have.

• 入館ゲートに学生証をかざしても開かない場合はカウンターにお申し出下さい。
  If the gate doesn't open with your ID, please speak to a counter person. 

• 連絡先等の情報に変更が生じた場合は、UTASでご自身で登録してください。
  Please update your registration information in UTAS by yourself if any of it has changed. 

• 医学部・医学系研究科、附属病院に所属している方は、原則として医学図書館がホームライブラリ（諸手続きや資料等受取の窓口）です。
  For those who belong to the school/faculty of medicine or the UTokyo Hospital, the Medical Library is your "Home Library", the library where you proceed with various procedures and receive the materials you ordered.

• 医科学研究所や新領域など別キャンパスの研究室に所属されている方は、ホームライブラリを変更することができます。希望される方は、医科学研究所図書室・柏図書館に手続きをご相談ください。
  If you belong to the lab in other campuses, you can change your home library to the Institute of Medical Science Library or to the Kashiwa Library. Please ask the IMSUT Library and the Kashiwa Library for necessary procedures. 

• 「ICカード職員証がまだ発行されていないが、すぐに図書館を使いたい」という方には、仮カードを発行いたします。フォームから申請してください。
  A temporary card will be issued if you want to use the library before you receive your IC card. Please apply via the web form. 

• 仮カードは、仮の番号・仮の期限で利用情報を登録するものです。
  The temporary card is for registering your information with a temporary number and a temporary period.

• ICカード職員証が発行されましたら、仮カードは回収し、改めて申請手続きをしていただくことになります。
  Once your IC card ID is issued, the temporary card will be collected and you will be required to apply again.

※仮カードをご利用の間も、MyOPACの各種サービス（文献取り寄せサービスなど）をご利用いただけます。
You can use MyOPAC services (such as a  delivery request service) while you are using the temporary card. 

• 「在籍を証明する書類」は、「在籍」と「在籍期間」を確認できる書類です。
  The "Enrollment Certification Documents" refers to any documents that can confirm your enrollment in UTokyo and the period of enrollment.

• 以下の項目は、在籍状況を確認するために必須の項目です。これらが確認できるものを提出してください。
  The following items are necessary to confirm your enrollment in UTokyo. Please submit a copy of a document that contains these items. 

◆必須項目 Necessary items: 

• 名前、所属部署、身分、在籍期間、発行部署名（印）
  Your name, department, status, period of enrollment, issuing department (and its stamp)

• 自身で判断がつかない場合は、お手元の書類を提出してください。
  If you are not sure about the documents, please submit a copy of any documents that you have and you think it may be suitable. 

• 病院の入退館カードは証明書にはなりません。
  The hospital entrance/exit card cannot be considered a certificate.

• ICカード身分証、その他在籍を証明する書類のいずれも持っていない場合も、このフォームから申請してください。
  Even if you do not have either an IC card ID card or other documents to certify your enrollment, please apply with this form.

• 当館担当者から、申請者が在籍している部署・講座・教室の担当者に連絡をとり、在籍や期間を確認します。
  The Medical Library staff will contact the persons in charge of your department / course / laboratory, to confirm your enrollment and its period. 

• ICカード学生証の方をご利用ください。手続きは必要ありません。（例：届出研究員かつ学生の方など）
  Please use your IC card STUDENT ID. No procedure is required.

• ICカード職員証での申請はできません（ICカード学生証が優先されます）。
  You cannot apply with your IC card staff ID, for your IC card student ID takes a priority. 

• この申請フォームにアクセスするには、Googleアカウントへのログインが必要です。
  This application form requires you to login with your Google account. 

• 下記いずれかのGoogleアカウントにより、ログインしてください：

  To learn more about MERCS Mail Service, please refer to the web page of the Office for Information Services, Graduate School of Medicine (on-campus access only). 

• 新規登録を希望する方、期限の延長など登録情報に変更が必要な方も、こちらのフォームからお申込みください。
  If you wish to newly register, or if you need to renew your registered information, please apply from this web form.

• このフォームは、図書館利用登録とMyOPAC登録の共通の申請フォームです。
  This web form is a common application form for using libraries and MyOPAC delivery request service.
