Excuse from regular PE

In order for a student to be excused from physical education, one of the following needs to happen:

1. A DOCTOR'S note needs to be turned in with

    • dates that student will not be able to fully participate

    • what the issue is

    • what the student CAN do

    • what the student CAN'T do

2. A PARENT may fill out the following form: This form is only good for ONE school day. A Doctor's note will be required for missing any more than that.
