Preliminary Hazard Analysis Report
Introduction
The hazards analysis process begins at the conceptual design stage of a project to assure that identified hazards are mitigated early in the evolution of the design. Resources available for the development of this Preliminary Hazard Analysis Report (PHAR) include but are not limited to prior project Hazard Analysis Reports, safety subject matter experts (SMEs) from conventional facilities, Engineering, and ES&H, and recent lessons learned. The Project ES&H Manager, with input from other ES&H Representatives, and SMEs, has to evaluate the Project’s risks against its execution plan for inclusion in the PHAR.
Document Requirements at Each CD Phase Gate
Prior to CD-1, prepare a PHAR to identify and evaluate all potential hazards and establish a preliminary set of safety controls. Hazardous chemicals are analyzed in accordance with Integrated Safety Management (ISM) requirements in DOE P 450.4A, 29 CFR 1910.119, and40 CFR Part 68.
Prior to CD-2, a Hazard Analysis Report is developed by updating the PHAR to include any new or revised information on facility hazards and safety design. If the hazard characterization is below Hazard Category 3 by analysis, the SBAA should approve this analysis before CD-2.
Prior to CD-3 and CD-4, hazard analysis and controls are updated in the Hazard Analysis Report.
