JCYC ETS forms

To enroll in JCYC ETS:

Please complete our intake form with your parent or guardian in either English, Spanish, or Chinese. For instructions on how to complete the forms, please click here.

JCYC ETS is funded by the Department of Education, and they require that we collect these forms from the students and families we serve.

Although our services are FREE and available to ALL students at Balboa, we are unable to show the Department of Education that we provided a service unless the student has completed our forms.
