Health Clearance (pdf)  |  Required by the Hawai‘i Department of Health (HAR Chapter 11-157) for certain health requirements to be cleared before students can enter into post-secondary institutions. Submit the completed, signed, and stamped/sealed form to Med+Proctor

Change of Major  |  Update your official declared major at Kapi‘olani CC
