According to the Transition Coalition, effective transition assessment should be a collaborative process that includes both the student and their family. It should be part of a broader assessment strategy, continuously updated, and used to guide transition planning. To begin this process, consider these six guiding questions:
What do we already know about the student’s strengths, preferences, interests, and needs related to employment, postsecondary education, and independent living?
What do we need to know about the student’s strengths, preferences, interests, and needs related to employment, postsecondary education, and independent living?
What methods and sources will we use to collect the information we need?
Who will gather the information, and what role will the student play in the assessment process?
When will the data be collected and used for transition planning?
Is the student making progress toward their measurable postsecondary goals (e.g., employment, education, living)?
