Initial meeting with student/family
Review past data and transition activities
Complete transition activities
Assess as needed along the way, collecting data in a central Google Doc or upload into VA IEP
Parent/Student final interview for IEP data analysis.
60 days before due date:
Call parent and schedule meeting
Complete and send Meeting notice and Agency Invite to parents
Send invite (and Zoom link) to all IEP team members
Begin entering Transition Assessments and Student/Parent Surveys
Send Teacher Input forms out to teachers and co-teachers
50 days before due date:
Compile all documentation and begin writing IEP
Upload Transition assessments to VA IEP if they are not already uploaded
40 days before due date:
Provide PA with IEP draft, Transition assessments, Teacher Input forms, and IEP Checklist for Review completed by You. Peer Reviews also acceptable.
30 days before due date:
Hold meeting
After meeting:
Ensure signatures are obtained no later than 1 week after meeting.
Send updated IEP at a Glance to teachers and co-teachers
Upload copy with signatures into VA IEP and file hard copy with signatures in file. If parent consent offered for Agency, upload that as well
Link All docs for that given IEP such as agency consent, assessments, and any additional documentation materials.
