Ongoing:
Initial meeting with student/family-record and build on it annually
Complete transition activities/services identified in IEP
Complete new transition assessments and collect other academic and functional assessments throughout the year.
Collect data and upload into VA IEP everything that will be used for the creation or defining measurable postsecondary goals.
Ensure student voice is active in all work. Include the full IEP team
60 days before IEP due date:
Call parent and schedule meeting
Complete and send Meeting notice and Agency Invite to parents
Send invite (and Zoom link) to all IEP team members, include any requested agencies.
Ensure Transition Assessments and Student/Parent Surveys are uploaded for consideration in final IEP creation.
Send Teacher Input forms out to teachers and co-teachers
50 days before due date:
Compile all documentation and begin writing IEP
Ensure PLAAFP includes summary of transition: identifies strengths, preference, interest, and needs in relationship to measurable postsecondary goals, and clearly stated status of Agency Connections,
Upload Transition assessments to VA IEP if they are not already uploaded
40 days before due date:
Provide Program Advisor with IEP draft, Transition assessments, Teacher Input forms, and IEP Checklist for Review completed by You. Peer Reviews also acceptable.
30 days before due date:
Hold meeting
After meeting:
Ensure signatures are obtained no later than 1 week after meeting.
Send updated IEP at a Glance to teachers and co-teachers
Upload copy with signatures into VA IEP and file hard copy with signatures in file. Upload Agency Request with final outcome (consented or denied).
Link All external docs for that given IEP such as meeting notice, agency consent, assessments, and any additional documentation materials to the finalized IEP (all related document should easily be acessible).
