In Hampton City Schools transition planning is 

an ongoing process of “Intentionality” 

In alignment with the Academy Model, transition supports students' learning continuum in becoming college, career, and life-ready. Transition planning has proven to increase the percentage of students with a disability who are:

• graduating with a standard or advanced diploma,

• increasing representation in colleges and universities,

• in desired employment earning a living wage,

• living independently, and 

• engaged as active members within their community.
