In Hampton City Schools transition planning is
an ongoing process of “Intentionality”
In alignment with the Academy Model, transition supports students' learning continuum in becoming college, career, and life-ready. Transition planning has proven to increase the percentage of students with a disability who are:
graduating with a standard or advanced diploma,
increasing representation in colleges and universities,
in desired employment earning a living wage,
living independently, and
engaged as active members within their community.
