Full Speech-Language Evaluation Procedures for Speech ONLY Concerns: Articulation and Fluency
Step 1: Referral for Full Speech-Language Evaluation
MTSS Coordinator makes sure that all items on the SPED Evaluation Submission Form are uploaded in Infinite Campus.
Step 2
School-based RTI Review Committee (RRC), including the MTSS Coordinator, SLP, and grade level teacher, review file using the RTI File Review Checklist section on the SPED Evaluation Submission Form for guidance.
Step 3
Once file is approved by the school's RTI team and the SPED Evaluation Submission Form is complete, email Mindy Watkins.
Use these guidelines for your email:
Subject: Student Name, Type of Referral (ex. John Smith, Full SPEECH ONLY Evaluation referral)
The body of the email should include student’s full name, student number, date of birth, and any other valuable information.
Step 4
Once approved, the student will be added to the Student Services Referral Log. Sherry Taylor will send an email to the team (MTSS coordinator, special education coordinator, lead special education teacher, SLP) stating the file is approved and a consent packet for a Full Speech-Language Evaluation is being sent to the school. School then sends packet home to parents.
Step 5
If the consent to evaluate is sent to the school, complete the portion at the bottom of the form indicating the date, the school name, and the individual at the school who received the form and return the form to Sherry Taylor immediately. When consent packet is received, Sherry will send the team another email.
Step 6
SLP will be logged to complete evaluation.
Step 7
Continue interventions and progress monitoring until the team meets to determine eligibility.
Step 8
SLP will set up an eligibility meeting within 60 days of the signed consent form.
Step 9
Team meets to determine eligibility.
