Step 1: Review Team Evaluation
MTSS coordinator and school-based RTI File Review Team will complete the Review Team Evaluation-Request for SPED Evaluation form. The following individuals at your school will complete the Observation/Feedback Summary: classroom teacher, EIP/intervention teacher, grade level RTI lead, MTSS Coordinator, instructional coach/school-based reading resource person, school-based math resource person, school-based behavior resource person, school counselor, school psychologist, speech and language pathologist, ESOL teacher, and gifted teacher.
Step 2: Referral for Special Education Evaluation
MTSS coordinator makes sure all items on the SPED Evaluation Submission Form are uploaded to Infinite Campus.
Step 3
School-based RTI Review Committee (RRC), including MTSS coordinator, school psychologist, RTI lead teacher(s), will review file using RTI File Review Checklist section on the SPED Evaluation Submission Form for guidance.
Step 4
Once file is approved and the School-Based RTI Review Team Evaluation-Request for SPED Evaluation and the SPED Evaluation Submission Form are complete, signed, and uploaded to Infinite Campus, Assistant Principal/Principal will complete the Google Form (submission)
Once the link is completed, the information will be sent to Mindy Watkins to process. If additional information is needed you will be notified.
Step 5
Once the SPED Evaluation Referral is received, the file will proceed through two review phases.
Phase 1: The file will be procedurally reviewed using the following checklist:
Intervention Areas are marked with no discrepancies in information across forms, email, etc.
All RTI forms are completed and uploaded to Infinite Campus.
Vision and Hearing Screenings are within 9 months of the referral date
Diagnostic Testing Protocols are within 9 months of the referral date
Exclusionary Factors section is complete
RTI File Review Checklist is complete and Intervention Plans are in place
All team members have handwritten signatures on the SPED Evaluation Submission Form, including the psychologist and SLP (if applicable).
If items are missing or incomplete, you will receive an email making you aware of what needs to be completed prior to the Phase 2 Review: File Review Team.
Phase 2: File Review Team
When the file is complete, the team will review the file. The desire of the File Review Team is to ensure there is evidence that a student is not responding to general education RTI interventions. The team will confirm there were at least two Data-Based Individualization (DBI) cycles at the Tier 3 level. The team will ensure appropriate screener/data was given and research-based interventions were chosen according to data and deficits. It will assist schools with planning instructional strategies that will differentiate on the basis of students’ varying skill levels in all intervention areas.
Step 6
Once approved, the student will be added to the Student Services Referral Log. Sherry Taylor will send an email to the team (MTSS coordinator, psychologist, diagnostician, special education coordinator, lead special education teacher, SLP (if applicable) stating the file is approved and a consent packet for a Special Education Evaluation is being sent to the school. School then sends packet home to parents.
Step 7
If the consent to evaluate is sent to the school, complete the portion at the bottom of the form indicating the date, the school name, and the individual at the school who received the form and return the form to Sherry Taylor immediately. When consent packet is received, Sherry will send the team another email.
Step 8
School psychologist and SLP (if applicable) will be logged to complete evaluations.
Step 9
Continue progress monitoring and documenting progress monitoring data in Infinite Campus every 2-3 weeks. Continue meeting every 4-6 weeks until the team meets to determine eligibility to review the progress monitoring data, discuss the student’s progress toward the goal(s) and evaluate the effectiveness of the intervention(s).
Step 10
Special Education teacher will set up an eligibility meeting - must be within 60 days of the receipt of signed consent form.
Step 11
Team meets to determine eligibility.
