Step 1
If a parent request, acknowledge parent’s request (request can be made verbally to a staff member, in an email, in a note, or in person). Set up a meeting with MTSS coordinator, teachers, parents, and your school psychologist. If there are communication concerns, invite your SLP.
Step 2
At the meeting, review the evaluation process with parents and discuss options: Tier 2 interventions, Tier 3 diagnostic testing and interventions, special education evaluation, 504. Clarify parent concerns and next steps. Set up intervention plans for students’ areas of concern.
(**If the team thinks it is necessary to proceed directly to the special education evaluation, don’t delay the referral to collect RTI data. The interventions need to be set up right away but can be implemented while you are submitting paperwork and the psychologist is evaluating.)
Step 3
Complete the Google Form (submission):
Once the link is completed, the information will be sent to Mindy Watkins to process. If additional information is needed you will be notified.
Next email the school principal, and the school psychologist with student’s full name, student number, and date of birth so that the consent for evaluation packet can be sent to the parent. Student will be added to the Student Services Referral Log.
Sherry Taylor will send an email to the MTSS coordinator stating a consent packet for a special education evaluation is being sent to the school. School then sends packet home to parents. The 60-day timeline for the evaluation report begins when the school (or anyone within the school system) receives the signed consent.
STEPS 4 THROUGH 7 SHOULD BE COMPLETED BY THE TIME PARENTAL CONSENT IS RETURNED TO THE SCHOOL.
Step 4
Compile all available items using the Evaluation Referral Checklist on the Simultaneous Tier 3/SPED Evaluation Submission Form (under RTI Documents tab). All the forms mentioned on the checklist can be created on the Documents tab under RTI on Infinite Campus.
Step 5
School-based RTI Review Committee (RRC) (include MTSS coordinator, school psychologist, RTI lead teachers) will review file using the RTI File Review Checklist on the Simultaneous Tier 3/SPED Evaluation Submission Form (under RTI Documents tab) for guidance.
Make sure to obtain the necessary handwritten signatures of the committee members who were in attendance at the file review meeting, including your school psychologist and SLP (if applicable), and upload to Infinite Campus.
Step 6
Use the Additional Information/Special Circumstances document (under RTI Documents tab) to indicate why the file is being submitted as a Simultaneous Tier 3/SPED Evaluation Referral (Parent Request for Evaluation).
Step 7
Once file is complete and approved by your school-based RTI Review Committee, reply to Sherry Taylor's acknowledgement/procedures email and Cc your school psychologist letting them know all documents are complete and uploaded to Infinite Campus.
The Simultaneous Tier 3/SPED Evaluation Submission Form, and Additional Information/Special Circumstances form should be on Infinite Campus.
Step 8
Student Services will verify all required documents are on Infinite Campus.
Step 9
If the consent to evaluate is sent back to the school, complete the portion at the bottom of the form indicating the date, the school name, and the individual at the school who received the form and return the form to Sherry Taylor immediately. When consent packet is received, Sherry will send the team another email.
Step 10
Screen student’s hearing and vision. Make sure passing hearing and vision (near and far) screenings are on Infinite Campus.
Step 11
Have teacher(s) complete Teacher BASC-3(s). Put Teacher BASC-3(s) in the yellow folder for diagnostician.
Step 12
Contact diagnostician for WIAT and WMLS-III (if any Spanish language background).
Step 13
After diagnostic testing is complete, schedule a Tier 3 meeting to review progress and modify intervention plan. Send home B7 notice to invite parents to Tier 3 meeting. Your school psychologist and SLP (if speech or oral language concerns are present) should be invited to the Tier 3 meeting.
Tier 3 meetings for parent requests are required every 4-6 weeks only if the school agrees the student has deficits that should be addressed through RTI interventions. Diagnostic screening results should support the decision to have/not have meetings every 4-6 weeks.
Step 14
School psychologist and SLP (if applicable) will complete evaluations.
Step 15
Continue interventions and documenting progress monitoring data in Infinite Campus every 2-3 weeks. Continue meeting every 4-6 weeks, until the team meets to determine eligibility, to review the progress monitoring data, discuss the student’s progress toward the goal(s) and evaluate the effectiveness of the intervention(s). Invite parents, psychologist, and SLP (if applicable) to Tier 3 meetings.
Step 16
Special education teacher will set up an eligibility meeting – must be within 60 days of the receipt of signed consent form.
Step 17
Team meets to determine eligibility.
*Please Note: Files are only considered Simultaneous Tier 3/SPED Evaluation referrals if the student has not been previously approved for Tier 3. Parents can request an evaluation at Tier 3, but it is not a Simultaneous Tier 3/SPED Evaluation request due to the student already being approved for Tier 3. It would be considered a SPED Evaluation Referral.
