DO I NEED TO FILL OUT THIS FORM?

If you are a student who will participate in a senior design activity which happens off campus, you must fill and sign the below release of liability and assumption of risk form. 

WHAT DO I DO ONCE I'VE FILLED IT OUT?

The properly filled and signed form must be delivered (email or physically) to the Academic Director of the program prior to the activity.
