Meet the team

Dr. Edilma L. Yearwood,  PhD, PMHCNS-BC, FAAN

Dr. Edilma L. Yearwood, Project Director for this HRSA grant is pleased to be working with our Washington, DC community of parents, providers and children to increase literacy around the social determinants of health and the potential impact of these factors on child well-being. We hope that by increasing knowledge, awareness and ability to access resources, we can all work together to improve the lives and overall health of the children in DC.  

Sarah Price, MSN-Ed, RN

Sarah Price has over 19 years of nursing experience, focused in community and public health. Sarah is currently the Director of Public Health Integration at NACHC and also an Adjunct Faculty for Public Health Nursing at Georgetown . She is an advisor on this Georgetown community initiative--identifying key community stakeholders/groups and providing community health center workflow context.

Margarita Sokolenko

As a research assistant on this project, I am honored to be involved in an opportunity to make a difference in the D.C. community. I am very passionate about helping the children and families of D.C. gain the resources they need to reach better health outcomes.

I've seen firsthand the hardships that my friends and families faced in their health journey, and I strive to be a force of change in combatting these disparities through this project.
