Transition planning is a process that helps students with disabilities prepare for life after high school. In Texas, transition planning is part of a student's Individualized Education Program (IEP) and should begin by the time the student is 14 years old. More Info HERE
When it begins
Transition planning should begin before the student is 16 years old, and is typically included in the IEP when the student turns 14.
Who is involved
The student, their IEP team, parents, and other invited participants are involved in the process.
What it includes
The student and their IEP team create a vision for the future, and then identify specific goals and services to work toward that vision.
What it helps with
Transition planning can help students decide what they want for their future, and figure out how to get there. It can also help the student's family and teachers understand more about the student's future.
