Arts Administration Override Request
The override request form should be used if a student is unable to register for an Arts Administration course. The course must have the "AAD" subject code. These requests can be made for the following circumstances:
A student has completed the required prerequisites but can’t enroll;
A student is requesting to be overridden into a course requiring pre-requisites because they have not met those prerequisites;
A student wishes to take a prerequisite course and the course for which that prerequisite is required simultaneously;
Prerequisites for a course change from one catalog year to the next;
The course is full; and
A student wants to take the course but is not an AAD pre-major or major.
Course Incomplete Request Form
Students experiencing significant hardship within the course of a semester may request an incomplete grade from the faculty of record for their course. The Course Incomplete Request Form is a contract between the student, instructor, and department that outlines the work left to be completed in the course and the deadlines for submission. Once your faculty member agrees that the student may request an "Incomplete" grade, the student and faculty member should agree on the following requirements:
What assignments/projects/exams are left to complete?
What is your agreed deadline with the instructor for completion?
Once gathered, the student should complete the Course Incomplete Request form. The request will be reviewed by the faculty member and department chair for approval. Students will be notified via email of the outcome of the request.

Arts Administration Major Application
Students are accepted to the BA Arts Administration as pre-majors until they meet the requirements to become majors.
To be admitted as an Arts Administration major each student must meet the pre-major requirements. Applications for admission must be submitted by the deadline in the semester prior to taking an upper-division Arts Administration course.
Arts Administration Petition Request
The petition request form should be completed for:
Course substitutions;
Required course exemptions;
Alternative minors/certificates or other non-departmental courses to count for directed electives; and
Transfer coursework that may not have previously been recorded.
Internship Forms
AAD 399 Learning Contract (On-Campus Internship)
AAD 399: Arts Administration Practicum is a university-based hands-on learning experience mentored by a university faculty or staff member. Practicums must be supervised by an Arts Administration faculty member. One credit hour is equal to 50 work hours. A maximum of 2-credit hours of AAD 399 can be applied towards a student's required 6-credits of internship hours. This is a controlled enrollment course and a completed Learning Contract is required. The following learning contract MUST be completed and signed by all parties before the first hour of work is completed. Before you begin completing the Learning Contract, you need to have your internship location and work assignment, approved by the Internship Coordinator or the Director of Undergraduate Studies. AAD 399 is pass/fail option only. The course may be repeated to a maximum of four credits.
AAD 499 Learning Contract (Off-Campus Internship)
All students in the BA of Arts Administration are required to complete 6-credit hours of internship by enrolling in AAD 499: Arts Administration Internship. Students must complete their internships at two different organizations. Each credit hour of internship is equal to 50 working hours (6 credit hours x 50 = 300 internship work hours). One internship MUST be completed at a nonprofit arts organization. The other may be completed at a for-profit arts organization such as a record label or radio station or a nonprofit organization (not arts-related). The following learning contract MUST be completed and signed by all parties before the first hour of work is completed. Before you begin completing the Learning Contract, you need to have your internship location and work assignment, approved by the Internship Coordinator or the Director of Undergraduate Studies.
AAD 699 Learning Contract (MA and GC Students)
All students in the MA of Arts Administration are required to complete 3-credit hours of internship by enrolling in AAD 699: Arts Administration Internship unless they are approved for an internship exemption. Each credit hour of the internship is equal to 50 working hours (3 credit hours x 50 = 150 internship work hours). The internship must be completed at a nonprofit arts organization.
The learning contract MUST be completed and signed by all parties before the first hour of work is completed. Once a student is offered an internship, they can complete the learning contract which will be forwarded to the following people for signature and/or comment:

Independent Study Forms
AAD 395/695 Independent Study Form for BA and MA Students
Students in the Department of Arts Administration may submit a proposal to complete an independent study. This form should be used for BA/MA students only. Please have the following information ready to submit:
Project Title.
Project Abstract. 100-word overview of the project.
Purpose. What will you accomplish by completing this project/study? What are your goals?
Description. What will the project/study entail?
Rationale. Why is the project/study important? What do you hope to learn from it?
Procedure. How will you go about doing this project/study? What are the deliverables? What is the timeline for project activities and deliverables?
Literature Review. A list of readings (bibliography) of at least 3-5 pertinent sources that will aid in your understanding of the primary topics/concepts that your project entails.
Supervisors. All site or faculty supervisors.
AAD 395/695 Independent Study Form
AAD 695 Independent Study Form for PhD Students
Doctoral students in the Department of Arts Administration may submit a proposal to complete an independent study. Independent study requests should align and support the student's dissertation research. Please have the following information ready to submit:
Project Title.
Project Abstract. 100-word overview of the project.
Purpose. How will completing this independent project aid in your dissertation research?
Description. What will the project/study entail?
Rationale. Why is the project important to your dissertation research? What do you hope to learn from it?
Output. What is the proposed output for the project?
Literature Review. An annotated bibliography of at least ten (10) pertinent sources.
Supervisors. Who is your faculty supervisor?
Timeline. What is the timeline for the project? Please include the frequency of meetings with your faculty supervisor.

Letter of Recommendation Request
Do you need a recommendation for an internship, job, scholarship, or graduate school? Your first step is to email the faculty member you are requesting a recommendation from.
Once you receive confirmation that the faculty member is willing to write a letter on your behalf, please complete the following form which will provide all of the required information to the recommender.

Please complete this form in order to update your communication preferences from the Department of Arts Administration. With this form, you can:
Update the email where you receive communications;
Update your preferred name;
Identify your preferred pronouns; and

Change Your Name for Department of Arts Administration Emails
The department maintains its own email system, so even if you change it using the steps above, you will still need to complete the Preferences for Department Correspence form to update your preferred name, preferred pronouns, or email address.
