IMMUNIZATIONS REQUIRED FOR SCHOOL ATTENDANCE K-5th Grade
State law requires children to be immunized against certain vaccine-preventable diseases to attend school in Texas. Getting your child vaccinated protects your child's health and that of the community.
Students cannot attend school without the appropriate documentation for the required vaccines or a valid medical or conscientious exemption.
DTP/DTaP - Minimum 4 doses required; one dose must be received on or after 4 years of age
Polio - Minimum 3 doses required; one dose must be received on or after 4 years of age
MMR - 2 doses required; first dose must be received on or after 1 year of age
Hepatitis A - 2 doses required; first dose must be received on or after 1 year of age
Hepatitis B – 3 doses required
Varicella – 2 doses required; first dose must be received on or after 1 year of age
For children on a delayed immunization schedule or those not receiving immunizations due to Reasons of Conscience please complete the Affidavit Request for Exemption from Immunizations for Reasons of Conscience at:

The Exemption form must be requested from the State and can take 14+ days to arrive through the mail. Immunization Records or the Exemption form must be turned into the front office by the first day of school.

WHERE TO GET IMMUNIZATIONS:
KINDER SELF CARE SKILLS – these skills should be taught at home and the child should be self-sufficient by the time they start kindergarten.

Medication Policy Guidelines:
The District shall not purchase oral nonprescription medication for students. In order for students to receive District-approved oral nonprescription medication (Tylenol, Advil, Benadryl and Tums,) while at school, the parent/guardian must complete and sign the Request for Administration of Medication form and the parent/guardian should bring the age-appropriate medication to the school nurse in the original container (smallest container available) and it must be properly labeled. The nurse may give up to 10 doses of the District-approved oral nonprescription medications. When students take 10 doses of a medication, the parent/guardian will be notified a doctor’s note will be needed to continue to give the student the medication.
All other nonprescription medication must have a doctor’s note and must be in the original container and be properly labeled. The parent/guardian must complete and sign the Request for Administration of Medication form and the parent/guardian should bring the medication to the school nurse.
All prescription medication that cannot be scheduled for other than school hours may be given at school. The parent/guardian must complete and sign the Request for Administration of Medication form and the parent/guardian must bring the medication to the school nurse. The medication must be in the original container and be properly labeled.
When the duration of the medication is completed, unused portions of the drug should be picked up by the parent/guardian. Any abandoned or expired medication not picked up by the parent/guardian at the end of the school year will be disposed of properly by Frisco ISD via an authorized medical waste management contractor.
A list of medications that are used in the school clinic for first aid is available on request from the school nurse. Please contact your school nurse with any questions.

All forms & medications must be turned in at the start of each new school year for the medical management for your child. Inform your child's teacher and school nurse of any pertinent information. Entering it into Info Snap at registration does NOT guarantee the info will get to the right person.
Request for Administration of Medication Form - all prescription medication, including district approved over-the-counter medication – acetaminophen (Tylenol), ibuprofen (Motrin), diphenhydramine (Benadryl) and Tums: Required medication including district approved over-the-counter medication must be supplied by the parent. All prescription medication must have the original prescription label attached. If the label is not attached, then you must have the physician fill out the paperwork and sign. District approved over-the-counter medication may be administered up to 10 doses. After 10 doses, the parent will be notified that a physician’s note/prescription will be needed to continue to administer the medication. This form must be completed every year.
Severe Allergy Action Plan – Asthma Action Plan – Seizure Action Plan: If your child has a medical diagnosis of a Life- Threatening Allergy, Asthma, or Seizures we prefer an Action plan and emergency medication in the clinic. A physician and the parent must fill this paperwork out so that I can better treat your child. This cannot be accepted without the signature from the physician. The Action Plans must be updated yearly.
Food Allergy Substitution Form: A physician and the parent must complete this form so that the cafeteria knows that your child cannot have certain foods. The cafeteria does not have access to your child’s food allergy information – the only way they will know about it is if they have this form on file. Unless there is a change in a food allergy, the form only needs to be completed & submitted once while enrolled with FISD. Account Restrictions Form: Dietary Restriction Request OTHER THAN FOOD ALLERGIES. This form is recommended to be completed so the cafeteria knows that your child cannot have certain foods. The cafeteria does not have access to your child’s dietary restriction information - the only way they will know about it is if they have this form on file. https://www.friscoisd.org/departments/child-nutrition/home – select Account Restriction – Elementary form
Food Allergy Information & Consent Form: Must be completed by the parent. Please communicate with your child’s teacher and the nurse regarding the food allergy and if you would like your child to sit at the Nut Free table during lunch. (Note: if your child has a nut allergy, this must be completed to have your child sit at the “Nut Free” table during lunch). This form must be completed every year.
Medication: I prefer to keep medication (Benadryl, Epi-Pen, inhalers with spacer/chamber) here for the year in case of emergency. Required emergency medication including any district approved over-the-counter medication (Tylenol, Motrin, Benadryl, Tums) must be supplied by the parent in the original container, FISD does not keep a stock of medication in the clinic. Prescription medications must have current prescription labels attached. Expired medication will not be accepted or dispensed. Please make sure that the Medication Administration form is complete. Due to Covid 19 outbreak, nebulizer medication will not be used in the school setting.
After the paperwork is proper filled out, it can be brought to the school or faxed directly to my office from the physician’s office. Medication may be dropped off at the front desk or the nurse’s office. FOR SAFETY REASONS – DO NOT SEND MEDICATION IN WITH YOUR CHILD AT ANYTIME DURING THE SCHOOL YEAR. Students are not allowed to carry any medication in their backpacks, lunchboxes or pockets.
WHEN TO KEEP YOUR CHILD HOME FROM SCHOOL:
Do not send your child to school for the school nurse to decide if they should be here – Refer to the above circumstances or contact your physician. If a child becomes ill while at school, faculty and staff do their best to isolate him or her and contact parents promptly for pick up. Please be sure to keep the front office up to date with any phone number and/or email changes.
CLINIC COMMUNICATION:
On average, the clinic sees 40-70 kids per day – this does not include those students who have regular scheduled medications, treatments or procedures. Every visit to the clinic is logged into an electronic record keeping system. My method of contacting parents may be with a phone call, email or a note sent home in your child’s daily folder. You will be called for emergencies, need for you to pick up, needs medical evaluation/X-ray/Sutures, Etc. Please ensure your phone number and email address on file are current.
You may not hear from me for skinned knees, abrasions, chapped lips, lost baby teeth, minor bumps/bruises – these are all normal parts of childhood. The teachers all have classroom boo-boo boxes to help take care of these minor issues in class and at recess.
The most common complaints are headaches, tummy aches and dizziness. It’s often from thirst, hunger, feeling tired or pushing themselves too hard. Most headaches and stomachaches will work themselves out on their own without any medical intervention. A student does not need to come to the clinic if their headache or stomachache just started. Usually, these ailments will resolve themselves in 30-45 minutes. If they are seen in the clinic, they will be assessed, drink water, allowed to rest and return to class when they are feeling better. It usually resolves quickly and the child goes on about their day.
Lots of kids like to come see the nurse, not all of them have a true medical issue going on. If an issue is determined to be mental health related, I will see if the school counselor can talk with them.
