Your child's teacher and I work closely together to keep your child safe and healthy at school. We consider ourselves part of the team when managing your child's diabetes and we value open communication and trust.
For students who have diabetes and who will seek care for diabetes at school, the parents and physician must provide a diabetic management and treatment plan for school. The parent shall provide all supplies necessary for monitoring and treatment/management of your child's diabetes as well as a diabetic plan from your doctor. The plan must be updated every school year. Our school's 504 Coordinator will contact you regarding setting up a 504 meeting for your child.
Diabetes Medical Management Plan: Local physician's/Children's Hospitals will have a printable Diabetes Medical Management Plans for school use. Please be sure the plan has the following information:
Diagnosis
Procedures needed-when to test blood glucose and precautions- treatment for hypoglycemia and hyperglycemia
Medication-Insulin dosage, amount, and type of delivery- insulin pump, pen, injection..etc.
Meal plan- Insulin to Carb Ratio, Insulin Sensitivity Factor, and target glucose
Provider's assessment of student's diabetic self-management skills
Recommended Diabetic Supplies for School - Supplies are to be replenished by parents as needed through out the school year:
Glucometer
Test Strips
Lancets
Alcohol wipes
Ketone Test Strips
Insulin (If your child is on a pump, we like to have insulin on hand in case of a pump malfunction)
Glucagon Kit
Pump Supplies - batteries, extra infusion set, insertion set & instructions
Treatment for low Blood Glucose - juice box, glucose tabs, quick acting carbohydrate snacks etc.
Extra snacks as needed
Storage container for supplies
Texas Legislation Bill (79R) HB984 states that an "Unlicensed Diabetes Care Assistant" designated by the principal and School Nurse will be instructed and trained in diabetes care. They may administer diabetes treatments, medication, and or procedures if a licensed health care professional is not available. The parent will be informed on the staff member that serves as the UDCA for their campus.

Cafeteria/Nutrition Department does not have access to your child's health information. If your child will be purchasing food items from the cafe, it is recommended the FISD Special Diet Accommodation form be completed to notify the cafe of your child's nutritional needs. This form only needs to be completed once while your child is enrolled in FISD. This form must be signed by a physician and parent. Once completed and signed by your physician it may be faxed, emailed or dropped off at front office or with the nurse.
