Immunization Information

Students must be current/on-schedule with their immunizations or have an Exemption form on file in order to attend school – this is a state mandated law.   

IMMUNIZATIONS REQUIRED FOR SCHOOL ATTENDANCE K-5th Grade  

•  DTP - Minimum 4 doses required; one dose must be received on or after 4 years of age

• Polio - Minimum 3 doses required; one dose must be received on or after 4 years of age

• MMR - 2 doses required; first dose must be received on or after 1 year of age                                  

• Hepatitis A - 2 doses required; first dose must be received on or after 1 year of age

• Hepatitis B – 3 doses required

• Varicella – 2 doses required; first dose must be received on or after 1 year of age

For children on a delayed immunization schedule or those not receiving immunizations due to Reasons of Conscience please complete the Affidavit Request for Exemption from Immunizations for Reasons of Conscience at:     https://corequest.dshs.texas.gov/ .  This form must be ordered from the State and it takes a few weeks to arrive through the mail – please plan ahead!
